Immediate Care

Because Spectrum Healthcare offers walk-in service, use this form to start the medical documentation of your pain or issues. We want to know if we will be seeing an adult or a minor, if there is a known pregnancy, if there are any drug allergies, and what medications (if any) are being taken. We must have your signed consent to begin treatment.

Physical Medicine

For those with planned appointments, use this form to let Spectrum Healthcare know your basic contact information, including insurance and your emergency contact information. We also want to know where you are feeling pain, and how intense it is. Tell us your known medication and food allergies (including the reaction), as well as your history of surgery, family health, and habits of nicotine and alcohol.
